This card is to be show to non-VA hospitals during emergencies if the veteran wants the VA nofitied for billing purposes. It is not required.

We are again making Ohio Veterans ID Cards for veterans living in Belmont County. Cards will be made by appointment only.  As required by Ohio Law, we must have a copy of your DD214, a photo ID(Ohio Drivers license or Ohio ID card) and a current bill with your name and current address to issue the card. This card is for Belmont County veterans only. Like all our services, there is no charge for the ID Card.
